4. Cancellation Policy – The Member must provide at least 24 hours notice to cancel a scheduled session. Failure to do so will result in a $5.00 cancellation fee or loss of that session. – The Trainer reserves the right to cancel sessions due to illness or emergencies] and will provide notice as soon as possible. 5. Health and Safety – The Member confirms they are in good health and physically capable of participating in personal training sessions. – The Member agrees to disclose any medical conditions or injuries that may affect their ability to participate in personal training. 6. Liability Waiver The Member acknowledges that participation in physical exercise carries inherent risks and agrees to hold the Trainer harmless from any claims, injuries, or damages arising from participation in the training sessions and has reviewed and signed the KDA WAIVER AND HOLD HARMLESS AGREEMENT. 7. Confidentiality The Trainer agrees to keep all personal information about the Member confidential and will not disclose any information without the Member’s consent. 8. Termination Either party may terminate this Contract with 30 days written notice. In the event of termination, any outstanding payments will be due. 9. Governing Law This Contract shall be governed by the laws of the State of Illinois 10. Entire Agreement This Contract constitutes the entire agreement between the parties and supersedes any prior agreements or understandings.
