NOW, THEREFORE, in consideration of being allowed to participate in the aforementioned activity, the
Indemnitor agrees as follows:
1. Waiver of Liability
The Indemnitor releases, waives, and discharges the Indemnitee from any and all claims, demands,
rights, and causes of action that may arise from participation in the activity, including but not limited to any
injuries, damages, or losses sustained.
2. Hold Harmless
The Indemnitor agrees to indemnify and hold harmless the Indemnitee from any claims, actions, or
liabilities, including legal fees, arising from the Indemnitor’s participation in the activity.
3. Acknowledgment of Risks
The Indemnitor acknowledges that participation in the activity involves inherent risks, which may include
but are not limited to physical injury, property damage, etc. The Indemnitor accepts and assumes all such
risks.
4. Medical Consent
The Indemnitor grants permission for emergency medical treatment to be administered in the event of
injury or illness during the activity. The Indemnitor agrees to be responsible for any medical expenses
incurred.
5. Destruction of KDA Property
The Indemnitor agrees to be responsible for any KDA destruction of property incurred.
6. Governing Law
This Agreement shall be governed by the laws of the State of Illinois.
7. Severability
If any provision of this Agreement is found to be unenforceable, the remaining provisions will continue in
full force and effect.
